Rough at idle & low speed

The past week I've noticed sometimes on start up the car runs rough, shakes a little, like a "put put put" sound. On pick-up it does it too, but around 40-50mph it stops and runs as normal. There are no warning lights or anything glowing. I do know there is a hole in the muffler that has been getting worse and makes some of the noice and vibration but I don't know if it would really cause this sorta issue or not...Not much to go on but just thought I'd ask if anyone had any thoughts...

plugs, wires, fuel pressure regulator, fuel & air filters? When was the last tune up and what was done?

I have no idea. The original engine failed last year and so a used one went in. Came with no maintenance records. I just know it's from a cared for car. Since that time there hasn't been much use, maybe a few hundred miles. 106,000 miles on the engine.
